Last 2 years W2's or tax returns
Proof of last 30 days' income (paystubs, etc.)
All pages of your last 2 bank statements for all acounts, including checking, savings, etc.
Proof of ANY liquid assets, such as a 401k
Statements pertaining to the debts you want to pay off
